No, losing a confidence vote in parliament (as opposed to in the Conservative party) would mean dissolving the parliament, not just Johnson stepping down, so the Tories would be unlikely to vote no confidence. However, it is still a threat that Starmer can use, since it would be at least inconvenient.
I thought losing a confidence vote meant Parliament had 14 days to choose a new PM that a majority of MPs would support and it only triggers a General Election if they fail to do so. There was some talk of using the mechanism to replace May's minority government with a temporary 'Government of National Unity' made up of Labour, Libs, SDP & Greens but they couldn't reach agreement on who would lead it. In this case if the Tories were willing to unite behind a leader, even a temporary one until they've held their leadership contest, they have the votes to maintain the rest of their term.
I'm not sure. If he'd lost a no-confidence vote by the MPs, then yes, he would be precluded from standing, but he hasn't.
