Well, you're free to think whatever you wish, of course, but it seems to me like a strange extrapolation once you even admitted that you don't know.
Lots of people choose their screen persona or market they catter for, or have it chosen for them, based basically on what works or what seems to be the low hanging fruit.
The lead vocals and composer of The Cure for example has an on-stage persona of someone deranged and depressive, and writes songs to match, but IRL he's not actually either. He's just milking a certain teenager market by selling them what they want to buy.
Alice Cooper has an on-stage persona of basically a raving lunatic, but if I'm to trust Randi who worked back stage with him, he's actually nothing like that off stage. Again he's just doing a choreographed show for the market segment that is into that kind of show.
Christopher Lee has been typecast into evil or creepy roles, but IRL he really is neither. It's just what his fans have come to expect to see him playing.
Queen's Freddie Mercury sang about fat bottomed girls making his world go round, and he was gay. Those girls didn't actually make anything go round or any other shape for him
Or to return to porn, there are a few women directors who depict women in demeaning situations, including for example Bridgette Kerkove who IIRC made a couple of movies in which she's getting anal gang raped. Probably _the_ nightmare scenario for most women out there, a lot more so than getting pissed on by Max. Heck, IIRC at least one where she's a schoolgirl getting gang raped, and that would probably make quite a few parents of both genders cringe. It doesn't really mean they have that low self-esteem and opinion about being a woman, but more likely reflects what they think about their male viewers' fantasies.
You just can't extrapolate from someone's over-the-top screen persona to their RL persona.
