The discussion about heart surgeons brings up a question: how do you even know a heart surgeon is a real qualified heart surgeon?
What's to prevent me, someone who never went to medical school, from just putting out a sign that I do heart surgeries for 2,000,000 Doritos (I'm assuming there is no traditional money with government being gone) and throw in some brain surgery for a crate of Hot Pockets? Oh I demand payment up front with no refunds and if anyone objects I use the hired security I have protecting me to "disappear them."
Heck, why would there even be medical schools in this type of world? Why actually study to be an engineer when anyone can just claim to be one?
..Sorry for jumping in late, but just worked through the entire thread and noticed this line of questioning was never answered, though the answer should be obvious to most whatever your view.
To be brief: there would always be a market demand for certification and a market demand for compatibility (in reference to your medical and currency questions). Arguably these markets would be much more evolved as there would be no tax funded state service to compete with. And unlike a state driven certification service, they *would* be economically vulnerable to charges of fraud.
Also many arguments have been made elsewhere that there would be a greater supply of medical practitioners. Its painfully obvious in my country that supply is nowhere close to demand for example.

If you don't mind going a little deeper into it, who would charges of fraud be filed with? Who would decide if they had merit? If I understand what you are saying, the only repercussion to fraud would be a loss of business. Is that correct?
What about lone practitioners that do not belong to a professional association, and can not therefore be penalized for malpractice by their peers? They can always claim whatever cewdwntials they wish to. What is their motivation to stop butchering people?
