This does not seem to limit it to DSD's but all women. See

So we are back to the sole criteria being hormone level regardless of cause.
As for definitions if they work they work, if they do not they do not. Kind of like how 9 planets is not a tenable number of planets for our solar system no matter how much people like Pluto and hate Ceres.
