Remember that quotation you posted earlier, which you said described a vicious circle? There seems to be some sort of relationship here, but it all seems to be part of the same syndrome. I don't think you're going to find the sort of simple causal relationship you seem to want.

Yes, it looks like that. But I think genetic/inherant predisposition may be; Ocassional, excess & irregular eating habits probably triggered by ocassional, excess & irregular gastric acid and insulin secretions-compounded by modern life-styles and environment. We can also think it by "Cephalic phase effect"--which a predisposed person may be experiancing more getting all these problems.
