It is very simple: the doctors simply stop accepting any and all insurance. It saves a lot of nonsensical office overhead in dealing with insurance companies (and especially MediCare and MedicAid) and so greatly reduces clinic costs. Win-win-win.

Anybody who believes doctors would cut off a lucrative deal for brittle ideological reasons is dense. Doctors I know are overjoyed, though many have very few spots for new patients as the demand has risen considerably.
