According to an article in Science (22 October) a design error by Lockheed Martin caused some sensors to be installed upside down, leading the spacecraft to crash into Utah last September (because they failed to sense the appropriate motion and therefore did not tell the parachute to open).

The sort of problem that requires testing and verification before deployment?

Anybody involved with the Star Wars anti-missle missles now emplaced in Alaska happen to read this? Especially the suggestion that rush to deployment could lead to sloppiness? Will anyone be embarrassed if one of those craft crashes into Utah?
