The Arizona State Forestry Division has released their 120-page report on the Yarnell Hill incident which points to suspected communication equipment failure or human error.
Findings include:
- a 30-minute period of radio silence up to the point the crew deployed fire shelters.
- the crew left the safety of previously-burned ground to enter unfavorable terrain after a wind change report was issued.
- aircraft loaded with fire retardant was orbiting nearby the incident but was unaware of the situation.
