Urine drug testing and prescription drugs

I recently was in ICU for 7 days as a result of a motor cycle accident.
The accident occured on the 18th Feb 2010
I had 5 fractured ribs close to where the ribs attach to the spine as well as internal bruising and soft tissue damage.

Here is a list of prescription drugs I have been taking.

Intravenous:
Morphine (Pain management)
Codeine (Pain management)
Tramadol (Pain management)
Pethedine (Pain management)
Intra dermal:
Lantus Insulin (Type 2 diabetes)
Oral:
Tramadol (Pain management)
Mypaid forte (Ibuprofen anti inflammatory)
Myprodol (Ibuprofen anti inflammatory)
Glucophage (Type 2 diabetes)
Simvastatin (Type 2 diabetes)
Gliclazide (Type 2 diabetes)
Herbal tea ( Nausea and constipation management)
Suppositories:
Arthrexin (non steroidal anti-inflammatory )


The intravenous drugs were administered during the ICU stay and the oral is what I have been taking since my discharge from hospital.

In 2 days time I must submit to my annual drug test, urine sample and multi drug test strip.

What are my chances of passing this drug test?

There may well have been some in the herbal tea though.

Watching Border on telly (Australian version) it seems that their electronic drug detector detects THC with heroine and cocaine contraband.

Any urine test I've given was accompanied by the question "what perscriptions are you on?" Do they not ask that anymore?

I am sure I will not have any problem as I have all the neccesary documentation, just interested to know if any of these drugs will be detected in the urine test as I do not know the biological half lives of these substances.

I think I saw that episode as well. I wouldn't fart anywhere near Customs if I were you.

Yep, they are quite radical at Australian customs. Some of the episodes are hilarious though.
There was one episode where a UK citizen, born in West Africa, married to an Australian woman gets hassled at the airport.

He only comes to Australia once a year and for the last 7 years, customs have singled him out for special attention. Gee whiz, did the black fella have a full go at customs and every camera he could find. His wife, a white lady was embarrassed out of her shoes

Any normal drug screen allows for you to list medications. The person collecting and processing the specimen may not be required to ask you for the information, but if you tell them, they have to record it. The MRO (medical review officer) will then take that into account when interpreting the results and reporting it to your employer.

Just come prepared with a full list of current and recent medications which might still be in your system.

You are going to fail for heroin and cocaine.

Would the cocaine result come from the codeine?
Does the ibuprofen result in a THC positive or would it be a false positive from the morphine/tramadol?

Don't know about tramadol, but it will come from morphine and codeine.

Morphine is the base unit of these drugs. Hydrolysis of heroin or codeine gives you morphine (although hydrolysis of codeine is not easy). Heroin and morphine, in particular, are very similar, and very easily interconverted.

Would the cocaine result come from the codeine?

Not in the least. They are unrelated.

I looked up tramadol. Depending on what markers they are using the tests, I suppose they could make a codeine connection, but I wouldn't expect it. It's a reach.

The only thing I think that might lead to cocaine detection would be the pethidine, but again, that depends on what markers they use. There's nothing in your list that is obviously cocaine-ish.

I'm going to predict you'll be clean.

The ITU meds: The morphine and codeine you had in ITU are opiate drugs (like heroin), so if you had been tested then, you would have had a positive for opiates. However opiates clear the system within days, so that won't be a problem. Tramadol and pethidine are opioids - this means they have a similar effect to opiates but are structurally different. They shouldn't affect a test for illegal drugs (and would have cleared by now anyway). Insulin is irrelevant.

Current meds: Tramadol as I've said is an opioid so shouldn't give a positive for opiates. None of the other drugs are related to drugs of abuse so I don't see why they would be a problem. Depending on how technically advanced the drug test is, they may be able to actually detect the prescription meds and tell what they are, but I can't see how any of them would give a false positive for abused drugs. Although it depends what's in that herbal tea

I do actually work in a lab that tests for drugs of abuse, though my knowledge is a bit rusty, so let us know how you get on!!

The herbal tea was given to me by a friend who has cancer and is undergoing chemotherapy.
He told me it worked well for nausea and as I was feeling nausea with the tramadol I gave it a go for about 2 weeks.

Sorted the nausea so I have a feeling there may have been some weed in it. I have had many drug tests in the past and have never tested positive for any drug, because, well I dont use illegal drugs.

I have the feeling that this one will be different.

I do recall reading that ibuprofen can cause a positive for THC. I have been eating those like sweets especially since the cold weather has set in.

I work for a consultancy in the nuclear industry and require access to nuclear installations.

Umm, I don't think you'll hit positive for cocaine but certainly for heroin. I don't know when you will be tested, but even these drugs clear the system fairly quickly but I imagine you are continuing pain management.
