Just saw an article about a man in the UK who climbed into a tiger's compound. Guess how that worked out for him. Definitely made my personal list.

Young man checked a muzzle-loading black powder pistol to "see if it's loaded." (Looked down the barrel while having his finger on the trigger.) It was loaded.
Sailor dove off the island of an LHA-class amphibious assault ship. NOT suicide, he though the nearly 100 foot dive was "doable". Two sailors tried to restrain him, he fought them off and did a running jump into the water. Water that wasn't there. He dove off the pier-side of the ship. SPLAT.
