Master Sasaki restricts himself to two kiai strikes on any one person at any one time. The risk of internal injury is just too great.
Yes, the person might burst his spleen laughing. :D

At the end, it would have been nice to see the "master" make the bell sound without stomping on the ground to make it vibrate.

That was interesting. I assume the ripple effect was added in post.

I wonder what would happen if he were attacked by a total stranger with a tire iron.

The mythology of various martial arts is full of this stuff. We have stories of the masters of "Kiajutsu" able to not only inflict injury with the voice alone, but to cure same.
This has been recycled in various fashions, even in fiction. In Frank Herbert's Dune, "The Voice" is used by the Bene Gesserit to control enemies and even cause damage.
 
A way kiai might work...

I guess shouting can be a neat trick. It may give you a split second during which your opponent will run the following thoughts thru his head: "WTF, what does she think she's doing, she's a chick and looks funny shouting"...so you use the WTF moment...
